An unusual gastric finding in a lung cancer patient - 2.º Prémio (Fotografia) Rui Mendo, Catarina O'Neill, Catarina Félix, Pedro Figueiredo, Luis Mascarenhas Centro Hospitalar Lisboa Ocidental A 44-year-old man with metastatic non-small cell lung cancer under pembrolizumab presented with 8-week course of progressive worsening bloating, dysphagia, vomiting and weight loss (10 kg). Esophagogastroduodenoscopy revealed, from the cardia to the pylorus, a diffusely swollen and friable gastric mucosa, covered with an off-white material due to exudate and sloughing. Biopsies revealed a mixed inflammatory infiltration of the lamina propria and glandular distortion associated with ulceration of the mucosa. Immunohistochemistry for Helicobacter pylori and cytomegalovirus detection were negative. Checkpoint-inhibitor induced gastroduodenitis was then presumed. Pembrolizumab was withheld and oral prednisolone was initiated, with a rapid symptomatic improvement. Outras fotos:Error
