A look into a ruptured pseudocyst of the pancreas causing life threatening haemorrhage - 1º Prémio (Vídeo) Catarina O'Neill, Joana Veloso Carmo, Catarina Félix, Rui Mendo, Cristina Chagas Centro Hospitalar Lisboa Ocidental 74-years-old man admitted due to abdominal pain and shock. He presented acute anaemia and bloody nasogastric aspirate. Medical history was relevant for a 10cm pancreatic pseudocyst. Urgent abdominal-CT showed cyst enlargement, intracystic haemorrhage and hemoperitoneum. Selective transcatheter arterial embolization of culprit branches of the gastroduodenal artery was successful, but five days later he presented recurrent upper gastrointestinal bleeding. Esophagogastroduodenoscopy demonstrated the pseudocyst had eroded the bulb wall and fistulised with spontaneous drainage of the pseudocyst/hematoma, probably due to duodenal wall ischemia secondary to embolization and increased intracystic pressure. This case illustrates a rare and potentially fatal complication of a pseudocyst.Error
