Gallstone ileus: A rare cause of bowel obstruction in a liver transplant patient

A 67-year-old man was hospitalized for suspected acute cholangitis. He had a history of liver transplant 20 years earlier and multiple hospital admissions due to acute cholangitis. Nevertheless, subsequent ERCP showed no evidence of dilated intrahepatic bile ducts or biliary stones. Over the following days the patient developed recurrent biliary vomiting. A computed tomography revealed dilation of jejunal loops and the presence of obstructing endoluminal content. A Double-balloon enteroscopy was performed revealing an impacted biliary stone with approximately 25 mm in the distal jejunum. The stone was removed using a retrieving basket (Roth Net®), with subsequent resolution of obstructive symptoms.
