An unexpected esophageal finding: esophagitis dissecans

A 57-year-old male was admitted to the intensive care unit with covid-19 infection, requiring invasive ventilation. Gastroenterology support was requested due to difficulty in placing a nasogastric tube, with a sensation of esophageal obstruction.
Upper endoscopy showed, at the level of the middle esophagus, whitish crepe-paper-like plaques that formed a pseudo-plug. After adequate distension, the lumen was permeable and longitudinal white plaques were adherent to the mucosa and were easily detachable, with the underlying mucosa being hyperemic. Biopsies revealed esophageal mucosa with inflammatory infiltrate, erosion and parakeratosis, confirming a diagnosis of esophagitis dissecans superficialis.
