An 83-year-old female patient with history of dysphagia, daily episodes of choking and regurgitation was admitted to our hospital due to a Takotsubo syndrome. During the first days, her clinical condition worsened due to aspiration pneumonia. Thoracoabdominal CT revealed bilateral pleural effusion and a large esophageal diverticulum. Esophagogastroduodenoscopy confirmed the presence of a larger diverticulum located in the middle-esophagus. After a multidisciplinary conference a decision to perform D-POEM was made.
Besides a high-definition video describing the different steps of this endoscopic procedure this case illustrates the challenges and the technical and clinical success of this minimally invasive endoscopic technique.
