A 57-years-old man was admitted for abnormal liver function tests. ERCP revealed an undetermined stenosis of the common bile duct (CBD) and a plastic stent was placed. During second ERCP, the stent fractured and a fragment migrated to the right hepatic duct. It was not retrieved after multiple attempts under fluoroscopy. The patient was referred to our tertiary centre for cholangioscopy which revealed an ulcerated perihilar CBD stenosis and an intraductal villous lesion of the mid CBD. Guided biopsies showed a low-grade-dysplasia adenoma. The foreign body was successfully captured and retrieved under cholangioscopic guidance using a novel SpyGlass Snare.
