Idiopathic chronic pneumatosis intestinalis: a rare incidental endoscopic finding

A 58-year-old man presented for a screening colonoscopy. Medical history was irrelevant and no history of abdominal trauma. Several polypoid grape-like submucosal masses, two of them focally congestive, were identified in the descendent colon. Since the lesions were easily indented with gentle pressure and had a bluish hue, pneumatosis intestinalis was suspected. The lesions were punctured with an injection needle (Endo-Flex®, Germany) and became deflated, confirming the diagnosis. The most frequent causes, including pharmacological ones, were excluded. This patient required no specific treatment, surveillance colonoscopies showed unchanged endoscopic features and he also remained asymptomatic during the entire 10-year follow-up.
