Underwater Endoscopic Mucosal Resection of a Lateral Spreading Tumor Overlying an Endoscopic Carbon Tattoo

A 53-year-old male with no relevant past medical history was referred to our department for resection of a lateral spreading tumor (LST) on the descending colon which was previously tattooed. On subsequent colonoscopy, a 40mm nongranular homogeneous LST was identified, overlying a previous carbon tattoo, with resulting fibrosis, that precluded conventional endoscopic mucosal resection (EMR). Therefore, underwater piecemeal EMR was performed, revealing involvement of almost all the submucosa by the ink. The resection was complete and without complications. Histopathological analysis revealed a tubular adenoma with low-grade dysplasia. On reevaluation, 3 months later, the scar had no endoscopic or histological recurrence.
