An 84-year-old female with past medical history of bladder cancer treated by transurethral resection presented with hematochezia. The blood analysis revealed microcytic anemia and the CT scan showed a diffuse thickening of the mid-transverse colon wall and colocolic intussusception. The colonoscopy revealed a large intraluminal ulcerated mass and signs of intussusception (photos). She underwent right hemicolectomy extended to the transverse colon. The postoperative course was uneventful. Histopathological examination of the specimen showed a large pediculate intermediate-grade adenocarcinoma of the colon (T3N0M0). Colocolic intussusceptions in adults are rare and almost always caused by a leading lesion which often warrants resection.
