A 24-year-old man, observed for lipothymia without any visible blood loss but with previous history of long-standing hypochromic microcytic anemia. A hemoglobin of 4.8g/dL was observed. Digital rectal examination was suspicious for the presence of soft small masses in the rectum. The patient was submitted to an upper endoscopy (normal) and colonoscopy with ileoscopy. On the colonoscopy more than 100 polyps with diverse appearance, dimensions between 5 and 30mm, with no endoscopic suspicion of malignancy. Final diagnosis of Familial Adenomatous Polyposis (index subject). The patient was submitted to a total proctocolectomy with transient ileostomy confirming multiple adenomas but without cancer.
