Atoll islands in a blue sea Catarina Correia, Elisa Gravito-Soares, Marta Gravito-Soares, Pedro Amaro Centro Hospitalar e Universitário de Coimbra A 70-year-old male performed a screening colonoscopy revealing colonic pandiverticulosis and a cecal 50-mm Paris 0-IIa, granular homogeneous LST lesion with no involvement of the appendicular orifice/ileocecal valve. Considering the lesion features suggesting no deep submucosal invasion, a piecemeal endoscopic mucosal resection was performed.<br />During the resection, six small islands of well-defined adenomatous lesions persisted. After a meticulous inspection of mucosectomy scar, it was possible to verify that these corresponded to lesion rings at the diverticulum necks surrounding normal diverticular sac fundus. Cautious additional resection of residual lesion was pursued. No complications. Histopathology showed a traditional serrated adenoma with low-grade-dysplasia.
