A 41-year-old female patient diagnosed with Peutz-Jeghers syndrome (PJS) experienced new onset of abdominal pain. After clinical evaluation she underwent a computed tomography scan that raised the suspicion of small bowel cancer. The study included various diagnostic procedures that didn’t confirm the initial hypothesis. Instead, the capsule endoscopy revealed the presence of a parasite typical of Ascaris lumbricoides, besides enteric polyps. Ascariasis is a worldwide parasite infection that is usually asymptomatic, when presenting with symptoms, abdominal pain is common. This case highlights the importance of capsule endoscopy in the evaluation of gastrointestinal symptoms in PJS.
