The hidden diagnosis in excavated gastroduodenal lesions Mara Sarmento Costa, Elisa Gravito-Soares, Marta Gravito-Soares, Pedro Figueiredo Centro Hospitalar e Universitário de Coimbra An 84-year-old woman presented to the emergency department for upper gastrointestinal bleeding with melena. She was previously diagnosed with a nasal melanoma with pulmonary metastasis, under palliative radiotherapy. At admission, she was conscious and pale. Heart rate was 87bpm, blood pressure 83/48mmHg and hemoglobin 6g/dL. After stabilization, she underwent upper endoscopy showing several voluminous excavated and ulcerated lesions in the gastric body and fundus and duodenum, up to the distal limit of the scope. Active bleeding was not identified. Histopathologic analysis showed proliferation of neoplastic cells with immunohistochemistry positive for SOX10 and Melan A confirming gastroduodenal involvement by melanoma.
