A 67-year-old male with no prior history presented to the gastroenterology clinic with recurrent episodes of rectal bleeding. The patient was previously asymptomatic and had normal laboratory parameters. He underwent a colonoscopy, which revealed an extensive, purplish lesion with focal aberrant vessels in the distal rectum, suggestive of hemangioma. A contrast-enhanced magnetic resonance imaging showed a 20-cm heterogeneous mass on the right anterior wall of the rectum, with high signal intensity on T2-weighted images. Given the most likely diagnosis, size of the lesion and probable recurrence of bleeding, surgery was proposed to the patient, who decided to postpone the procedure.
