An uncommon cause of dysphagia
André Coimbra Trigo; Dário Gomes; Pedro Narra Figueiredo
Centro Hospitalar e Universitário de Coimbra
A 69-year-old man presenting with dyspnea due to a pleural effusion underwent a CT exam which showed a retroperitoneal mass, highly suspicious of a lymphoproliferative disorder. Due to a solid food dysphagia, an esophagogastroduodenoscopy was performed, revealing in the lower two thirds of the esophagus a bulky, polypoid endoluminal formation with 10cms of extension, occupying the entire lumen, friable and ulcerated at the top. Histology revealed a lymphoid neoplasm made up by large B lymphocytes, with irregular nuclei and a high apoptotic and mitotic índex, expressing Bcl2 and CD23. Later, the patient was diagnosed with diffuse large cell B lymphoma.
