A bad gut-feeling: Kaposi’s sarcoma as an unusual cause of dyspepsia

A 29-year-old male infected with Human Immunodeficiency Virus infection on Highly Active Antiretroviral Therapy (HAART) for four years (CD4 count 371 cells/mm3, undetectable viral load) was submitted to an esophagogastroduodenoscopy (EGD) for epigastric abdominal pain and nausea, without other accompanying symptoms. On EGD, several erythematous and violaceous lesions, with variable sizes and appearances (flat, maculopapular, or polypoid) were detected. Histology exhibited submucosal proliferation of spindle-shaped cells and vascular slits containing extravasated erythrocytes; staining was positive for CD34 and HHV-8, being highly suggestive of Kaposi’s sarcoma. The patient was continued on HAART and liposomal doxorubicin was initiated; gastrointestinal symptoms gradually remitted.
