Intramural esophageal dissection: endoscopic septum incision

A 26-year-old female with eosinophilic esophagitis with persistent complains of dysphagia underwent upper endoscopy. Intramural esophageal dissection between the 25 and 28cm from the incisors was present. Despite histologic remission and optimization of medical therapy, patient had persistent dysphagia. Fluoroscopy during endoscopic reassessment revealed contrast retention inside the false lumen, inducing bulging into the true lumen. It was hypothesized as the cause for persistent dysphagia and endoscopic incision of the septum with a hook-knife (Endocut, Effect 2) was performed. No complications occurred. Patient was discharged home on the same day on oral diet. During follow-up, patient reported symptomatic improvement.
