A 57-year-old woman with a history of long-term heavy smoking was referred to the emergency department due to persistent vomiting after a traumatic brain injury. On admission, the computed tomography scan of the head showed multiple focal lesions resembling metastases. Further investigation identified a lung mass pointing to a tumor, although the fine needle aspiration cytology was inconclusive. Later on, due to suspected gastrointestinal bleeding presenting with melena, an esophagogastroduodenoscopy was performed. Several violaceous volcano-shaped and friable ulcers were found throughout the gastric body and biopsies were taken. The immunohistochemistry led to the definite diagnosis of non-small cell lung cancer.
