Intraductal Papillary Mucinous Neoplasm of the pancreas with gastric fistula - Menção Honrosa (Fotografia)

An 88-year-old man underwent upper digestive endoscopy for asymptomatic anemia with transfusion requirement. The endoscopy showed a bulging of the antrum’s great curvature causing pyloric deformation and a lesion of the posterior/small curvature of the body with exteriorization of a column of thick mucus very suggestive of pancreatic intraductal papillary mucinous neoplasm (IPMN) with gastric fistulization. After vigorous aspiration of the mucus it was possible to access a cavity covered by villous tissue. A computed tomography confirmed the endoscopic diagnosis and anatomopathological evaluation was suggestive of villous lesion with high-grade dysplasia compatible with an intestinal type of IPMN.
