Diaphragm disease: Non-Steroidal Anti-Inflammatory Drug - Induced Enteropathy

"An 82  years old female patient recurred to the emergency  room with chest pain. Patient was taking piroxicam for joint pain. Evaluation revealed Haemoglobin 3g/dL, requiring 9 units of RBCs. Bidirectional endoscopies were normal.  Video capsule endoscopy was performed..

FIGURE 1: Erosions with circular distribuition.

FIGURE 2: Circunferencial ulcer associated with a diaphragm-like stricture.

FIGURE 3: Extensive ulcer associated with stenosis. This figure correspond to the tenth and last stricture. Capsule didn't reach caecum during recording time.

Plain abdominal X-ray performed 2 weeks after the procedure confirmed capsule expulsion.

Diagnosis: "diaphragm disease",  pathognomonic  feature of NSAID-induced small bowell strictures.
