Hybrid-ESD of a recurrent superficial esophageal cancer

A 11mm superficial esophageal epidermoid cancer was diagnosed in a 59 years-old man that 3 years before had synchronous ENT and esophageal superficial cancer treated by radiochemotherapy and EMR, respectively. The endoscopic diagnosis was suggestive of submucosal invasion but, in spite of absent regional or distant disease, esophagectomy was excluded due to severe co-morbidities. Endoscopic resection was then performed by an hybrid technique: circumferential incision and subtotal submucosal dissection were done with a ClearCut-knife 2mm I-type (Finemedix) and en-bloc resection was completed with diathermic snaring (15mm oval snare). Pathology confirmed complete resection with deep sm2 invasion; adjuvant treatment was proposed.
