Distal to the Bulb, a False Tulip

A 61-year-old female with Sjogren's Syndrome was admitted in the Emergency Department with hematochezia. Upper endoscopy and colonoscopy were negative for the cause of hemorrhage. Capsule endoscopy suspected a protruding lesion in the proximal jejunum. Double balloon enteroscopy (DBE) confirmed the presence of a subepithelial lesion in the jejunum, with extensive ulceration of the surface mucosa.

Segmental bowel resection was performed. The pathological report showed a gastrointestinal stromal tumor (GIST).

Neoplastic lesions are an uncommon cause of small bowel bleeding. In some cases, the valorization of the capsule endoscopy findings with DBE was necessary for the diagnosis.
